- 1In the form wizard, open the questions step.
- 2Add the fields you need: name, email, phone, multiple choice, and others from the type list.
- 3Mark required fields. Optional fields belong below the must-haves.
- 4For email or phone, turn on OTP verification (email, SMS, or WhatsApp where offered) if you need confirmed contacts.
- 5Use help text under a question when the answer format is easy to get wrong.
OTP questions
Phone OTP can send SMS or WhatsApp depending on the question settings. Email OTP sends a code to the address they typed. People cannot submit until the code checks out.
Do not OTP every field. Verify email or phone, then ask the rest in plain questions.
Preview the public form before you publish — especially OTP screens.
